Face & Neck Contouring in London

A minimally invasive procedure designed to tighten and contour the skin beneath the chin and along the jawline. The treatment can be performed as skin tightening alone, or combined with targeted removal of excess fat to further improve definition and contour where required.

The Procedure

What face and neck contouring actually is

The jawline is a contrast effect. It looks sharp when there is a clear step between the edge of the mandible and the soft tissue below it. Two things blur that step: fat sitting in front of the platysma muscle, and skin that no longer retracts around the shape underneath. Treating one without the other is why so many patients are disappointed by a single-modality treatment.

Face and neck contouring addresses both in the same sitting. Through two or three access points — one under the chin, one behind each earlobe, each a few millimetres — the area is first infiltrated with tumescent local anaesthetic. Excess fat is then removed with fine microcannulae, deliberately conservatively: the aim is to restore a defined border, not to strip the neck, which looks hollow and ages badly.

Energy is then delivered to the undersurface of the skin. Controlled heating of the fibroseptal network causes immediate contraction of the soft-tissue envelope and triggers a collagen remodelling response that continues for several months. This is what allows skin to redrape over the new contour rather than hang off it — and it is the step that distinguishes contouring from liposuction alone.

The whole procedure takes between one and two hours. You are awake and comfortable throughout, and you walk out the same day.

Quantum, FaceTite, NeckTite and Endolift are the platforms used to deliver this energy. The rest of this page describes the procedure rather than the device, because the right platform depends on your anatomy, skin thickness and the amount of fat present — and that choice is made at consultation, not before it.
Suitability

Who this procedure is for

It tends to work well if

  • You have fullness beneath the chin or along the jawline that does not shift with weight loss or exercise.
  • Your jawline was once defined and the definition has softened over time.
  • Your skin still has reasonable elasticity — it does not need to be perfect, which is the point of the tightening component.
  • You are at or near a stable weight.
  • You want a meaningful change but cannot take two or three weeks away from work.
  • You would rather avoid a general anaesthetic.
  • You are in your thirties, forties or fifties — the range in which fat and early laxity coexist.

It is not the right procedure if

  • The fullness is caused by a large or ptotic submandibular gland, or by anterior digastric muscle bulk, rather than fat. Removing fat around these makes them more obvious, not less.
  • There is substantial skin excess — a true platysmaplasty or neck lift will give a result that this cannot.
  • Your neck fullness is largely subplatysmal, which is assessed on examination.
  • Your weight is actively changing, or a significant loss is planned.
  • You have a pacemaker or implanted metallic device in the field, an active infection, or a bleeding disorder.
  • You are hoping for a facelift result without a facelift. It will not deliver that, and no honest practitioner will tell you otherwise.

Deciding which group you fall into is the single most important part of the process, and it cannot be done from a photograph. The assessment examines skin quality and recoil, whether the fat is pre- or subplatysmal, the position and size of the submandibular glands, platysmal banding on animation, the cervicomental angle and chin projection. Where another procedure would serve you better, you will be told so.

On the Day & Afterwards

What to expect

The procedure

  • Photographs and markings taken while you are sitting upright.
  • Local anaesthetic to the access points, then tumescent infiltration of the whole treatment area.
  • Microliposuction of the preplatysmal fat with fine cannulae, crosschecked by pinch test as you go.
  • Energy delivered beneath the skin to contract the envelope, with continuous temperature monitoring.
  • Access points left to heal, and a compression garment applied.
  • Two to three hours. You are awake, and go home on the same day.

Recovery

  • Compression garment for 1 to 3 weeks dependent on the treatment performed — most of it at night after the first 3 days to 1 week.
  • Swelling and bruising for one to two weeks; the first few days are the worst of it.
  • Most patients return to work at two to seven days.
  • Numbness and firmness under the chin for several weeks, which settles.
  • Early contour change is visible once swelling subsides; tightening continues over four to six months.
  • Exercise from two weeks, building back gradually.
Case Studies

Patients treated by Dr Stark

Every case below is a patient of Dr Stark's, photographed in the same position and lighting before treatment and at the interval stated. All are single-session results. Images are published with express written consent in accordance with GMC guidance. Results vary between individuals, and the photographs are shown to illustrate what this procedure does — not to predict your own outcome.

Case 01
Softened jawline with submental fullness
6 weeks apart · 1 treatment

Loss of jawline definition with fat sitting in front of the platysma and early laxity of the overlying skin. Treated in one session with microliposuction of the submental and jawline fat, followed by energy-based skin tightening to contract the soft-tissue envelope.

Case 02
Early under-chin heaviness
4 weeks apart · 1 treatment

A younger patient with localised preplatysmal fat and good skin quality. A deliberately conservative microliposuction, combined with energy-based skin tightening, to define rather than hollow.

Case 03
Heavy lower face and blunted neck angle
6 weeks apart · 1 treatment

Fullness through the lower face and submental region with an obtuse cervicomental angle. Microliposuction was used to re-establish the border of the mandible and energy-based skin tightening applied to allow the skin to redrape over the new contour.

Case 04
Lower-face heaviness and a softened neck skin laxity
6 weeks apart · 1 treatment

Shown from the front: heaviness through the lower face with loss of a clean mandibular border and early skin laxity. Treated with energy-based skin tightening alone, contracting the soft-tissue envelope across the lower face and neck so the jawline reads as a defined edge again, without removing volume.

Case 05
Male patient — submental fullness
3 weeks apart · 1 treatment

A male neck with preplatysmal fat obscuring the jawline. Thicker male skin and a denser fibroseptal network were taken into account when planning energy delivery. Treated with microliposuction and energy-based skin tightening in a single session under local anaesthetic.

Case 06
Loss of definition along the mandibular border
3 weeks apart · 1 treatment

Fat accumulation beneath the chin with skin that had begun to lose recoil. Conservative microliposuction restored the step between jaw and neck, and energy-based skin tightening addressed the envelope above it.

Case 07
Fuller neck with a soft jawline
4 weeks apart · 1 treatment

Greater volume of submental fat requiring a more extensive microliposuction, with energy-based skin tightening to contract the overlying skin and avoid the emptied appearance that fat removal alone can produce.

Case 08
Skin laxity without significant fat
6 weeks apart · 1 treatment

A neck in which laxity rather than fat was the dominant problem. Treated with energy-based skin tightening alone — no liposuction was performed, as removing fat here would have worsened the result rather than improved it.

Case 09
Softening along the jawline and upper neck
6 weeks apart · 1 treatment

Loss of definition along the mandibular border with early laxity of the skin beneath it, and little excess fat to remove. Treated with energy-based skin tightening alone, contracting the soft-tissue envelope so the skin redrapes over the existing contour rather than removing volume from it.

Questions

Frequently asked

Is it painful?
The local anaesthetic injections sting for a few seconds. After that the area is numb and patients typically describe pressure and movement rather than pain. Simple analgesia is enough afterwards.
How is this different from liposuction alone?
Liposuction removes fat. It does nothing for the skin above it, so in anyone with even moderate laxity the result can look emptier rather than tighter. Adding controlled energy beneath the skin contracts the envelope at the time of treatment and drives collagen remodelling afterwards, so the skin redrapes over the new contour.
How is it different from fat-dissolving injections?
Injectable fat reduction works gradually over several sessions, treats fat only, and gives less predictable contour control. This is a single procedure with direct, tactile control of how much fat is removed and from where — and it treats the skin at the same time.
How long do the results last?
Fat cells removed do not return. The skin continues to age, so the tightening component is not permanent — most patients hold their result for several years. Weight gain will change the contour.
Will there be scars?
The access points are a few millimetres and sit under the chin and behind the earlobes. They are generally difficult to find once healed, but like any skin incision they are permanent.
When will I see the result?
An early change is visible as swelling settles over two to six weeks. The photographs on this page are taken at three to six weeks, which is why they look like a real recovery rather than a finished one. Tightening continues for four to six months.
Can it be combined with other treatments?
Yes, and it often is — contouring restores the border, while volume restoration or skin-quality treatment addresses what sits above it. The plan is made at consultation and staged sensibly rather than all at once.
Do you offer FaceTite, NeckTite or Endolift in London?
Yes. Dr Stark treats patients across four London clinics and uses Quantum, FaceTite, NeckTite and Endolift, selecting the platform at consultation rather than in advance. He is also an InMode trainer, teaching other doctors these procedures. Patients searching for FaceTite London, NeckTite London or Endolift London are almost always describing the procedure set out on this page.
Who performs the procedure?
Dr David Stark MD MRCS, a plastic and reconstructive surgery registrar and Member of the Royal College of Surgeons, with over 1,400 logged surgical procedures. More about Dr Stark →
